Health Form: New sports physical required every TWO years. However, form must be filled out and signed by the doctor and a parent EVERY year, and must be submitted to the CYO office before the athlete begins to practice or attends camp EVERY year.)

Eligibility & Exception Form (Required for any kids who want to play for a region other than the one closest to their home or school.) Athletes are NOT allowed to play or practice with a team UNTIL they are notified that their exception was approved by the Rules and Compliance Committee.)

Injury Incident Report To be completed by Certified Athletic Director, Coach or Site Official and should be filed within 24 hours to the CYO Office.

General Incident Report To be submitted by Certified Athletic Director, Coach or Site official. Used for violations of the "CYO Code of Conduct" and should be filed within 24 hours to the CYO Office.
