Coaching Application Form(must be filled out for first time CYO coaches only)

Eligibility & Exception Form(for kids who do not attend a Catholic school or parish, or for kids that want to play for a different region. Kids are NOT allowed to play or practice with a team UNTIL they are notified that their exception was approved by the Rules and Compliance Committee.)

Health Form (must be filled out and signed by the doctor each year)

Injury Incident Report To be completed by Certified Athletic Director, Coach or Site Official and shoule be filed within 24 hours to the CYO Office.

General Incident Report To be completed by Certified Athletic Director, Coach or Site official for violations of the "CYO Code of Conduct" and should be filed within 24 hours to the CYO Office.
