COACHES CERTIFICATION Click on "Coaches" to the left for details on coaching requirements. All coaches who volunteer with CYO must be certified. Cheer coaches may have other requirements as well. Please contact your Region Athletic Director for more information.

Eligibility & Exception Form (REQUIRED for kids who want to cheer for a region other than the one closest to their home or school. Kids are NOT allowed to cheer or practice UNTIL they are notified that their exception was approved by the Rules and Compliance Committee.)

Registration Form for Mail or Fax Online registration is preferred, so if possible, please use the link to registration on the main "cheerleading" page. However, if you are unable to register on line, you may download this registration form, PLUS the "Participant Registration Information Form" PLUS the "Health Form." You must complete ALL THREE forms and mail or fax them to the CYO office with your payment before you may check out a uniform or begin practicing.

Health Form (A new sports physical is REQUIRED every TWO years. However, a parent and your doctor must fill out and sign this form EVERY year, and the form must be received by the CYO office BEFORE YOU MAY BEGIN PRACTICING).
