Eligibility & Exception Form
REQUIRED for kids who want to cheer for a region other than the one closest to their home or school. Kids are NOT allowed to cheer or practice UNTIL they are notified that their exception was approved by the Rules and Compliance Committee.)

Health Form
A new sports physical is REQUIRED every TWO years. However, a parent and your doctor must fill out and sign this form EVERY year, and the form must be received by the CYO office BEFORE YOU MAY BEGIN PRACTICING.

Injury Incident Report To be completed by Certified Athletic Director, Coach or Site Official and should be filed within 24 hours to the CYO Office.

General Incident Report To be submitted by Certified Athletic Director, Coach or Site official. Used for violations of the "CYO Code of Conduct" and should be filed within 24 hours to the CYO Office.
