FORMS THAT YOU MAY NEED DURING THE SEASON:
Coaches 25 Point Rule Form If you outscore a team by more than 25 points you are required to complete this form and submit it to your athletic director who will turn into the CYO office.
Injury Incident Report To be completed by Certified Athletic Director, Coach or Site Official and should be filed within 24 hours to the CYO office.
General Incident Report To be completed by Certified Athletic Director, Coach
or Site Official for violations of the "CYO Code of Conduct" and should be filed within 24 hours to the CYO office.
FORM FOR CHILDREN THAT DO NOT ATTEND REQUESTING CATHOLIC SCHOOL OR PARISH:
Eligibility and Exception Form(for kids who do not attend a Catholic school or parish, or for kids that want to play for a different club. Kids are NOT allowed
to practice or play with the team UNTIL they have been notified that their Exception was approved by the Rules and Compliance Committee!)
